2025 Annual Community Report

A little about who we serve:

· We provided 25,575 visits to 5,334 children and adults last year for Medical, Dental, and Behavioral Health needs. 3,265 of these were virtual.

· The patients we serve are comprised of 26% Medicare, 29% MaineCare, 29% private Insurance, and 16% uninsured. We provided sliding fee discounts and other supports of more than $123,795, supporting 34% of our patients who fall at or below the 2024 Federal Poverty guidelines. A little about how we measure up:

· EHC continues to maintain NCQA Certification for the Patient Centered Medical Home Model and reports regionally & nationally on many quality of care measures. Here are a few of those quality measures for 2024:

o Breast Cancer Screening: 70% of our female patients have been screened for Breast Cancer

o Statin therapy for Prevention of Cardiovascular Disease: 82% of our patients with cardiovascular disease are receiving Statin therapy.

o Patients enrolled in Care Management increased by 54%, with 2642 patient encounters.

o Children aged 3-17 with Nutrition and Physical Activity Counseling: 68% of this patient population have received this counseling. A few things that we are proud to share from this past year:

· EHC is in growth mode. We continue to add new patients every day.

· We are building our care management services to support patients beyond the provider visit. This adds a layer of patient care for our patients.

· We are now distributing over 3,000 pounds of free produce monthly to community members.

· We welcomed Becca Starr, PA-C; Steven Parker, PA-C; Bill Richardson, MD; John Cassidy, PA-C, Jordan Brooks, RDH; and Laura Wakeman, LCSW this past year.

· We are in the design phase of a new EHC Eastport facility to be located at 75 Key St., Eastport! EHC’s Annual Public Meeting is June 24th at 5 PM. Please call 853-6001 for more information.
